Is Coronary Heart Disease a Complication of Diabetes?

Diabetes and coronary heart disease (CHD) are closely related. Diabetes is an independent risk factor for cardiovascular diseases, and CHD is one of the macrovascular complications of diabetes. In a sense, diabetes and CHD represent different stages of disease, forming an upstream-downstream relationship. Numerous epidemiological studies have confirmed that the risk of cardiovascular and cerebrovascular diseases in diabetic patients is 2-4 times higher than in non-diabetic populations. Elevated fasting and postprandial blood glucose levels significantly increase the risk of cardiovascular and cerebrovascular diseases even in the pre-diabetic stage.

Diabetic patients often also have hypertension, dyslipidemia, and smoking habits, which are multiple risk factors for cardiovascular and cerebrovascular diseases, further increasing the incidence of CHD in diabetic patients. In recent years, the "endothelial injury response hypothesis" has gained support among scholars regarding the pathogenesis of CHD. This hypothesis suggests that various risk factors ultimately damage the arterial intima, with atherosclerotic lesions forming as an inflammatory-fibroproliferative response to intimal injury.
